What are the correct immunization and physical forms for my child to attend Kids Discovery?

The Department of Children and Families require both a properly executed Florida Certification of Immunization Form, DH680 (dated July 2010), and a Student Health Examination (physical) Form, DH3040 (dated July 2013).

How do you monitor who enters and leaves the building?

The main entrance to the building is locked in between drop-off and pick-up times. Parents are required to use a code to gain access to the building. Once in the lobby, we have two locked doors that lead to our classrooms. To clock their child in and out, parents/guardians use their individual codes, which release the doors to allow entrance. Children are released only to those authorized by parents. Anyone unknown to our staff who requests to pick up a child is required to produce a photo ID.

What is your sick policy?

Any child with a fever of 101 degrees or higher and another contagious symptom (including but not limited to a rash, diarrhea, or sore throat) is not permitted to stay at the center. If your child is sent home due to an illness, he/she must be absent from the center the following day, at a minimum.

What are drop-off hours?

You may drop off your child between 6 and 10:30 am. If children are going to be dropped off later than 10:30 am, prior arrangements are required.

What ages do you accept?

Kids Discovery is open to children ages 6 weeks through 12 years old.

What do I need to do to withdraw my child?

We require a one-week written notice before disenrolling your child.

Do you administer medication?

Whether over-the-counter or prescribed, all medicine must be in its original container and labeled with physician’s name, child’s name, medication name and specific directions and dosages. All prescription and non-prescription medication shall be dispensed according to the label’s directions. All medicine, including diaper ointment, will not be dispensed for a period longer than two weeks unless we receive written instruction from a doctor.
